Elegant Flash Mail Forms Needed (available in: Long Island)

I'm looking for a beautiful Flash form that is both easy to read and very IMPRESSIVE looking. They can be identical (except for the informational fields). One form is a "Contact Us" form (probably 5 fields) and the other form is to collect preliminary information from potential customers (probably 10 fields).

- I'll need an HTML version of both forms for non-Flash visitors.

- It is important that the forms are secure and have anti-spam measures built in.

- The font used should be both really nice looking and easy to read...this font is really nice, but it looks fuzzy:

- Simple sound effects as an option would be great (i.e. form loading, typing, sending) - but should not be "over the top".
